The NOS example Michael posted is the correct one.
One thing to keep in mind if buying a tractor one. They work just fine. However the original carrier ones are two wire with an insulated socket. The tractor ones are a single wire with the socket grounded. I believe the LWD Parts lamp body is also single wire. Interestingly my Otter used the same lamp and mount however with the Hamilton Bridge Company part number (A163) and 42 stamped for the year. The Otter uses the single wire lamp.
