For Peter:

On the normal Canadian MKIII plug, pins 9, 10, 11 & 12 are not used. Not sure about your key rotation. Early plugs were fixed but later shells could be rotated.

For Mike:

It sounds like you have a connector for use with the British HP amplifier. The 6 pin versions were #14 & #16 and the 12 pin version was #27. The 2 pin lead went to the socket marked "control" on the front of the amp.
