The AAB417A Permanent Record Book for A Vehicles (the ones on the AWM site are AB417A's from a few of the Centurions) contain 12 sections:
1. summary of veh mileage, hours run, fuel & oil consumption
2. record of transfers between units
3. record of inspections and classifications
4. record of official mods
5. workshop repairs, overhauls, by date
6. brief particulars of the vehicle (engine no., etc)
7. main engine history (lists engine changes, serial numbers etc)
8. charging set engine history & mods
9. winch engine history (applicable to ARV, bridgelayer)
10. barrel history
11, breech ring history
12. breech mechanism history
The AWM examples will have unused pages in each section (they are rarely, if ever completely filled out, if at all: some sections never seem to be used much), so it would be possible to have them copied and that could form the basis of a 'tailored' log book for your purposes.
Otherwise, make it up yourself using a table in MS Word, print, and bind.
